Juniper Publishers| Forced Air Warming Device Failure Resulting in Smoke and Soot on a Surgical Patient

Image
Journal of Surgery -  JuniperPublishers Introduction Forced air warming (FAW) devices are used in the operating room to helpmaintain normothermia during anesthesia and prevent complications of hypothermia. Proper use involves connecting the hose of the warming unit to a disposable blanket with a perforation pattern that evenly distributes heat across the patient’s body. Multiple studies have shown the efficacy of FAW devices to be better than other widely used interventions such as circulating water mattresses [1]. However, as with most medical interventions there is a potential for complications with FAW devices. We present the case of a FAW device that malfunctioned after it became wet, depositing black soot on the patient. Go to Case Report A 67 year old male patient with a history of hypertension, dyslipidemia, and lung cancer presented for a robotic-assisted left upper lobectomy. The patient was taken to the operating room where standard ASA monitors were applied. Gener...

Juniper Publishers| Superior Mesenteric Vein Thrombosis as a Complication of Crohn’s Disease: An Unusual Case

Image
Journal of Surgery -  JuniperPublishers Abstract Background:  Mesenteric venous thrombosis (MVT) is a rare but potentially catastrophic clinical complication, which may lead to ischemia or infarction of the intestine. Methods:  We report here a case of superior mesenteric vein (SMV) thrombosis in a 21 year old male patient with no history of Crohn’s disease or other risk factors of thrombosis. Patient was admitted to the hospital with the signs and symptoms of small bowel obstruction. He was taken to the operating room after the initial diagnostic tests and underwent partial small bowel resection followed by end to end anastomosis. İntraoperative diagnosis was acute mesenteric ischemia (AMI). Results: Final pathological evaluation of the resected specimen was confirmed as demonstrating features of Crohn’s disease. Conclusion:  Crohn’s disease should be considered as one of the underlying causes when AMI is the diagnosis. Keywords:  Crohn’s disease; Acute ...

Juniper Publishers| Laparoscopic Resection of Large Gastric Gastrointestinal Stromal Tumor - Case Report and Concise Review

Image
Journal of Surgery -  JuniperPublishers Abstract Gastrointestinal stromal tumors (GIST) are relatively rare and commonly involve the stomach. Surgical resection is generally recommended when feasible. Although laparoscopic resection has been used for smaller tumors, resection for larger tumors is less well described. We report a case of gastric 6.5 cm GIST treated with laparoscopic wedge resection and review the literature. Keywords:  Gastrointestinal Stromal Tumor; Laparoscopic resection; Gastric tumors Go to Introduction Gastrointestinal stromal tumors (GIST) are relatively rare tumors affecting the alimentary tract, commonly the stomach. Treatment is surgical resection and laparoscopic resection is being increasingly utilized. We report a case of gastric 6.5 cm GIST treated with laparoscopic wedge resection. Go to Case Report 53-year-old gentleman was admitted with small bowel obstruction secondary to adhesions. His past medical history was remarkable for left orchie...

Juniper Publishers| Pseudomonas Infections in Surgery Practice Needs for Innovative Antimicrobial Procedures?

Image
Journal of Surgery -  JuniperPublishers Mini Review From environment to patient Pseudomonas genus is known and studied from many years and it’s currently widely to the attention in microbiological and in medical clinical area for severe infections occur in patients who are already illness or who have a weak immune system, such as long term hospitalized subject. This bacteria group, normally is isolated in the environment, such as in soil, water, plants and in nosocomial area as well as in veterinary field such as intensive no controlled farming [1]. Pseudomonas spp. represents a Gram negative opportunistic pathogens bacteria group with a great, outstanding adaptability in the different environmental conditions. In fact, recentstudies by using the comparative genomics on different strains of P. aeruginosa isolated in human and veterinary field have identified some key genes involved in the shift environment-status vs. pathogen-status of these bacteria. In particular, these st...

Juniper Publishers| Acute transverse colon volvulus with partial rotation

Image
Journal of Surgery -  JuniperPublishers Abstract Transverse colon volvulus is an uncommon entity practically very few experienced in their life time. This condition presents most common as emergency with investigation not diagnostic and surgery is the treatment of choice with high mortality and morbidity. This is the first case report of transverse colon volvulus with partial rotation, the following case illustrates and signifies of this condition and outcome in acute abdomen.  Go to Introduction Transverse colon (T-Colon) volvulus is a uncommon and first reported by kallu in 1917 [1] and till now 70 paper published till now to our knowledge as per PubMed search. The incidence of colonic volvulus is 3-5 % and T-colon volvulus represents 3-4% out of it [2]. Few of the papers published were in pediatric but it is more common in adult [2-4]. The mortality rates are three time more common in T-colon volvulus when compared to sigmoid or cecal volvulus. We report one case of ...

Juniper Publishers| A Review of Surgical and Endovascular Repair of Coarctation of the Aorta

Image
Journal of Surgery -  JuniperPublishers Abstract Coarctation of the aorta (CoAo) is a narrowing of the aorta in proximity to the ductus arteriosis that occurs in 1 of 2500 live births. It is a life-threatening condition that can present suddenly in neonates and infants. Despite great efforts, it remains one of the most difficult cardiac anomalies to diagnose prenatally. When severe, immediate surgical intervention is necessary at birth. Sometimes the condition is latent and patients are asymptomatic until childhood or adulthood. In this paper we explore the various interventions for infants and adults and discuss the stages at which surgical or endovascular techniques are recommended. Also included is a case report of a rare adult presentation of CoAo with concomitant cardiovascular disease. We discuss the possibility of treatment with 1-stage repair as well as introduce 2-stage repair as a new method for treating such patients. Go to Introduction Coarctation of the aorta (C...
