Juniper Publishers| Dengue Fever Outbreak in Surgical Patients: Diagnostic Challenges and Outcome Impact, 2014

  Journal of Surgery -  JuniperPublishers Abstract Background:  Dengue fever has re-emerged as a major arboviral infection in tropical countries. Its presentation in surgical patients has not been described where it becomes a diagnostic dilemma. Methods:  Six inpatients were diagnosed with Dengue fever in Surgical Gastroenterology ward in October and November 2014. All were evaluated with serial blood investigations and managed accordingly. Results:  Three of 6 patients developed Dengue Hemorrhagic Fever (DHF). Two patients succumbed to the illness. Definitive surgery was prolonged in another two. Conclusion:  Dengue fever should be suspected in all patients with postoperative rise of temperature, particularly in endemic regions.   Journal of Surgery -  JuniperPublishers Abstract Introduction:  Many benign colorectal polyps cannot be positioned correctly or reached by the endoscope for polypectomy. Accordingly, patients undergo surgical resection of involved bowel. Due to this fact, we have attempted to develop minimally invasive techniques in porcine models to assist with endoscopic polypectomies. The MiniLap is a novel device that deploys a grasping mechanism through a needle-tipped shaft. We used this device for dissection, exposure, retraction, as well as transcolonic insertion to assist in this endeavor. Methods:  Under IACUC approval, using a supine porcine model, two 12mm ports are placed (periumbilical and right lower quadrant) for a camera and instrumentation. Minilaps are inserted to assist with exposing, retracting, and dissecting.   Journal of Surgery -  JuniperPublishers Abstract Introduction:  Reoperative Abdominal Surgery (Relaparotomy) means an unplanned reintervention carried out during the immediate postoperative period after laparotomy and causally related to first operation. Method:  The study had been carried out in upgraded department of General Surgery in SMS Medical College and Hospital. The cases comprises of operated in SMS Hospital and those referred from other surgical centers. Results:  Maximum patients 70% needs relaparotomy only one time which had minimum mortality (11.4%). Four patients (8%) were needed relaparotomy 3 times in which 2 patient were expired. Mean while mortality rate of 3rd relaparotomy was 50%. Relaparotomy were done mainly for adhesion obstruction and anastomotic leakage.
